Youth Event Registration & Permission Form
TO BE COMPLETED ONLY BY A PARENT OR GUARDIAN (only youth over 18 can complete this form for themselves). Granted for both one-off and recurring transportation and event permission of a youth by Youth Unlimited | YFC Highlands staff or volunteers to allow for the youth to participate in programs. We are planning an activity as part of our programming that requires your permission prior to participation. We have provided you the details of the activity and request that you complete and sign this permission form. The safety of the youth is our primary concern. Precautions will be taken for their wellbeing and protection. Youth Unlimited YFC Highlands is collecting and retaining this personal information for the purpose of enrolling your youth in this activity. This information will be maintained indefinitely as it is a requirement of our insurance company and legal counsel. If you wish Youth Unlimited YFC Highlands to limit the information collected, or to view your child’s information, please contact us. This form should be completed by the Parent/Caregiver if under 18 or by the individual participant if 18 years or older.
Program Details
Alpha Weekend Celebration. Marcy is hosting two-day overnight weekend event in Grand Bend at 33992 Water Works Road from Friday August 7th until August 9th, 2026. Car transportation is provided from Huntsville to Grand Bend, plus for excursions over the weekend, and to return to Huntsville. Meals are provided and youth participants are asked to bring $100-150 for personal expenses (like for snacks or shopping). Activities will include walking, biking, shopping, and possibly a beach afternoon (weather dependent).
